The Therapeutic Effect of Cervical Medial Branch Blocks for Chronic Neck Pain

Abstract
To investigate the therapeutic effect of cervical medial branch blocks for chronic neck pain from the zygapophysial joints. Between March and November 2002, We retrospectively studied 50 patients (24 men and 26 wemen; mean age, 51 years) who had pain in one or more cervical zygapophyseal joints from various disease entities. A total case of sixty-eight medial branch of cervical dorsal rami blocks were performed for this patients. All the injection were in combination with lidocaine and methylprednisolone. The procedure was repeated in patients whose pain recurred after first injection within a couple of weeks after. the outcome was measured by the four point pain perception scale (excellent: pain free, good: improvement £¾50%, fare: improvement£¼50%, and poor: no change) and checked at 2 weeks and 1, 3 months after procedure. At two weeks follow up examine 8 (16%) and 27 (54%) of 50 patients experienced a excellent (E) and good (G) response to medial branch blocks. 7 (14%) and 8 (16%) patients experienced fare (F) and poor (P) response to medial branch blocks. There were 17 (34%) patients who treated the repeat procedure. At 1 and 3 month follow up result were E: 6 (12%) and 3 (6%), G: 23 (46%) and 24 (48%), F: 10 (20%) and 11 (22%), P: 11 (22%) and 12 (24%). Based on this study, medial branch of cervical dorsal rami blocks in combination with lidocaine and methylprednisolone was considerable therapeutic effect for chronic neck pain by one time or more treatments compared with radiofrequency thermocoagulation neurotomy.
